At Dr. Mize General Dentistry we are in network with the following insurance plans:

We are committed to helping you maximize your insurance benefits and we accept assignment of dental benefits from most insurance plans. However, keep in mind that your specific policy is an agreement between you and your insurance company. You are responsible for the total obligation should your insurance benefits be less than anticipated. We will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be. The fees charged for services rendered to those who are insured are the usual and customary fees charged to all of our patients for similar services. Your insurance policy may base its allowances on a fixed fee schedule, which may or may not coincide with our fees.

Payment Options

We accept cash, personal checks, Care Credit and major credit cards including Visa, MasterCard, Discover and American Express. Payment is expected at the time services are performed. If more extensive dental services are necessary, financial arrangements can be made with our office.
